VtmR uses a fork of QeRadiant to import custom level geometry into the game. This works for the most part but if you want to use terrain or something complicated you need to rely on outside programs. Terragen works with the level editor out of the box. But there's another tool that works really well as well.

Once you've extracted it and run it. Simply convert your mesh to obj and use the tool. I've found both standard and extrusion work however you will usually need to split up your mesh into smaller pieces if it's really complicated. I've gotten both Blender's Suzanne and more traditional meshes imported using this method.
